For decades, the character design of Two-Face has been pretty straightforward: his appearance split down the middle, with one side terribly scarred from an acid incident and the other left unmarred. However, one DC artist flipped this classic design on its head, drawing inspiration from a legendary manga artist.

The parasitic, tumor-like mass on Harvey’s head and the grotesque scarring evoke the visceral, unsettling imagery Ito is known for.

However, instead of following the traditional design, with one half showing the unmarred, handsome face of former district attorney Harvey Dent and the other depicting the scarred visage of one of Batman’s most iconic rogues, Artgerm takes a page from iconic horror manga creator Junji Ito’s playbook. The result is by far one of Harvey’s most disturbing and badass designs to date.

While Artgerm’s Two-Face is still scarred on one side, the damage is depicted very differently from the traditional version. The scarring nearly erases all facial features on the left side, with thick cords of tissue completely obscuring his eye. The most striking change, however, is the addition of a tumor-like mass on top of his head. This growth carries the classic elements of the Two-Face persona: a bulging eye, a twisted sneer, a shock of discolored hair, and heavy scarring. By presenting the “Two-Face” side as a grotesque extension rather than a split down the middle, the design more effectively portrays Harvey and Two-Face as separate identities.

This redesign is particularly effective in visually conveying Harvey’s Dissociative Identity Disorder. Rather than simply dividing his face in half, Artgerm presents the Two-Face persona as something growing out of Harvey, representing the constant mental battle between two competing personalities. One acts almost like a devil on his shoulder, always present but distinct. During his at MCM Comic Con 2025, Artgerm explained, “... usually when people interpret Two-Face [he] would be half-and-half, right? So, I want to make something slightly different. I want to make his other half of his personality manifest as his own.”

At the , Artgerm openly shared the inspiration behind his Two-Face redesign, stating, “It was originally inspired by Junji Ito’s work… so I could make it look [like] a demon itself.” Junji Ito, the legendary Japanese horror manga artist known for works like Tomie, Uzumaki, and Gyo, is celebrated for his mastery of body horror. Artgerm absolutely captures that influence in his version of Two-Face. The parasitic, tumor-like mass on Harvey’s head and the grotesque scarring evoke the visceral, unsettling imagery Ito is known for. While this interpretation departs dramatically from the traditional look, there’s no denying it’s one of the most visually striking and horrifying versions of Two-Face to date.
