It can be incredibly daunting to unearth every secret in managed to by players unnoticed. Since it's widely believed that every "Hidden Gem" has been discovered, actually tracking them down is now a little easier even if the process is overwhelming.
Over on Nexus Mods, creator MadCatMkII shared their "Hidden Gems Map" mod which, as the name suggests, treats Hidden Gems as a trackable objective. Players can track the secrets and Easter eggs scattered around Night City as though they were standard quests: The icon on the map will indicate where to uncover the find, which can then be followed with a custom map marker. The mod is also flexible in what it highlights as players can tweak it to display secrets, Easter eggs, Hidden Gems, unique items, curious events, or all of the above.

The Hidden Gems Map mod was developed with Cyberpunk 2077 and the Phantom Liberty expansion but the creator does note that some DLC items may not display correctly. If this is the case, they're open to players submitting anything that should be added to a database that can be found on the mod's Nexus page, ensuring that everything is eventually ed for. It's a fantastic project, one that will likely be a major boon for players who can't spend hundreds of hours wandering around the city or simply want to locate a specific secret.
For clarity, Hidden Gems can represent anything from loot to fun references to other media franchises. For example, the Fallout reference in the desert outside Night City is considered a Hidden Gem since it's not marked on the map as an objective but it's clearly still a point of interest. According to those who've managed to track down them all, there are around 190 "Hidden Gems" throughout the game, so having some guidance in tracking them all day feels almost necessary.
Those who'd rather take their time and organically uncover every Hidden Gem can do so but it'll likely take dozens of hours at the very least. Fortunately, CDPR is only in the very early stages of development on Cyberpunk 2077's sequel, so those dedicated fans have plenty of time to scour the world. There's always something to do in Night City, especially when it's off the beaten path.
