7 Billy Kincaid
Spawn Brutally Kills a Deranged Child-Murderer
Billy Kincaid was a child murderer who Al Simmons (before he became Spawn) was actually contracted to kill, before Billy was arrested and sent to prison. However, Kincaid only ended up serving six years in jail before he was back on the streets, and this twisted villain didn't waste any time in getting back into his old ways. Using an ice cream truck to lure children, Kincaid killed and mutilated the kids he kidnapped, and it seemed like he'd be able to do so with impunity forever. However, Spawn quickly tracked Kincaid down, and he put an end to his murder spree once and for all.
Spawn didn't just kill Kincaid, either. Spawn made Kincaid feel the pain and terror that he made all those children feel before killing them. Not only that, but Spawn incorporated an ice cream scoop and popsicle sticks into Kincaid's brutal murder before leaving his lacerated corpse hanging from the ceiling by Spawn's chains.
6 Clown/Violator
Spawn Uses a Necroplasm Explosion to Decimate the Clown/Violator
The Clown, aka the Violator is the oldest 'villain' in Spawn's life. The word 'villain' is in quotations because Clown was actually meant to help Spawn complete his mission that was given to him by Malebolgia, the King of Hell, who turned Al Simmons into Spawn. Spawn was supposed to use his symbiote suit to kill evil people, sending their souls to hell to fight in Malebolgia's army. Spawn may have been ridding the world of evil, but he was serving a dark, otherworldly cause with every villain he murdered (like Billy Kincaid). And Clown was meant to ensure Spawn was doing just that.

However, the Violator hates Hellspawn, and decided to actively make Spawn's life as miserable as possible every moment he could. That was the foundation of their relationship throughout Spawn mythos, and it finally came to an ultimate head in Spawn #300, when Spawn finally killed the Violator with a massive necroplasm attack that quite literally rocked the entire world - and was insanely brutal.
5 Byron
Spawn Cosplays as an Alien Chestburster for This Kill
While the overarching story arc of the early Spawn comics (which spans the first 100 issues) is Malebolgia using Spawn to grow his army in hell by recruiting every damned soul Spawn kills on Earth, there are plenty of smaller-scale storylines within that era, including ones that feature Spawn simply taking on street gangs - albeit high-tech and incredibly dangerous ones. One such gang was called The Nerds, and that gang had a cyborg enforcer named Byron. After a gang war involving Spawn, Byron found himself as the last remaining member of The Nerds, and decided that it was his time to rule the streets. Spawn had other ideas.
Using the otherworldly power of his symbiote suit, Spawn went inside Byron only to explode out of his chest the same way a Chestburster bursts out of its hosts' chests in the Alien franchise. Indeed, Spawn basically cosplayed as a Xenomorph for this kill, and it was just as brutal and bloody as it is in the Alien franchise.
4 Demon Kincaid
Billy Kincaid Came Back as a Demon, & Spawn Brutally Kills Him Again
Spawn does a thorough job killing the child-murderer, Billy Kincaid, when he got him the first time (as previously covered), but that was far from the end of Billy's story in Spawn mythos - in fact, it was only just the beginning. Truth be told, Billy Kincaid barely stayed dead at all in Spawn, as he was shown wandering through hell after Spawn murdered him, only to receive a symbiote suit of his own as a member of Malebolgia's army. However, Kincaid wouldn't make his fully demonic transformation for some time, but when he did, he was actually a challenge for Spawn himself.
In King Spawn #3-4, Billy Kincaid returns as a hulking demon with spikes protruding from his rotten flesh, and tattoos mocking Spawn and his previous child victims littering his body. Given his new status as an absolute beast of a villain, Demon Kincaid is able to throw Spawn around like a rag doll. However, that doesn't last very long, as Spawn uses his fists to beat Kincaid into a gory puddle of blood and bone.
3 The Freak
Spawn Kills the Freak by Literally Turning His Own Evil Against Him
The Freak is an interesting Spawn antagonist, as he's as utterly deranged as he is brilliantly manipulative. The Freak was a regular human who built a 'mansion' (of sorts) for himself in the sewer, from which he operated. The Freak's initial interaction with Spawn resulted in the Freak tricking Spawn into killing his therapist, before the Freak lured Spawn into the Dead Zone (where Spawn's powers don't work), where he crucified and decapitated Spawn.
However, it wouldn't be long before Spawn regenerated, and he made the Freak pay for all the evil he had committed in his life - and Spawn even weaponized that very evil itself. Spawn harnessed the concentrated darkness within the Freak's soul and transformed it into hordes of vermin - spiders, rats, etc. That manifested evil then tried to crawl back into the Freak's soul, resulting in the Freak's horrific death, as thousands of rats, snakes, and spiders forced their way into his body any - and every - way they could.
2 Malebolgia
Spawn Decapitates the King of Hell
Malebolgia, the main villain of the first 100 issues of Spawn, finally gets what's coming to him in the 100th issue, as Spawn finally has enough of being used and manipulated by the King of Hell, and decides to take the fight to him. Up until this point, Malebolgia was pulling the strings of Spawn's entire life. He sent the Violator to ensure Spawn never stopped killing evildoers, he manipulated those around Spawn to further push Simmons into darkness, and he quite literally used every 'good' thing Spawn had ever done against him in the most twisted and corrupt ways imaginable.
Indeed, Malebolgia was Spawn's arch enemy in those early books, which makes his brutal demise so satisfying. When Spawn (and Angela) go down to hell to kill Malebolgia, the battle is fierce, and Angela even loses her life. However, in the end, Spawn does what he came there to do. Spawn cuts off Malebolgia's head after the King of Hell is eviscerated by Angela, and screams an animalistic, cathartic battle cry as he holds the demon's head in the air.
1 Paul Greene
Spawn Tortures a Twisted Vampire to Death
Paul Greene isn't a villain as noteworthy as Malebolgia, or even the Violator, but he's easily just as twisted and evil, and more than worthy of Spawn's wrath. In the more recent Spawn comics, Spawn meets a woman who had been held captive by Paul (who is a vampire) for four years, and who even had Paul's child, who had been raised in captivity as well. Paul tortured this woman in every conceivable way for years, and after Spawn got her and her baby to safety, he made sure Paul suffered a fate that matched his twisted crime.
Spawn held Paul captive in an old cabin in the woods, hanging him from chains for days as he starved. Spawn then gave Paul the chance to escape through a tiny 'hallway' with spikes protruding from the walls. Paul ripped his own body to shreds trying to escape, but when he reached the other end, Paul only found his own dogs being controlled by Spawn, and the dogs had been starved as well, meaning Paul was on the menu.
But, Spawn gave Paul yet another chance to escape; he just had to go back through the spikes, or be eaten by his own dogs - making this probably the most brutal Spawn has ever been.
